What is Annex IV?
EU AI Act Annex IV (Article 11) defines the mandatory technical documentation that providers of high-risk AI systems must create and maintain. It covers 9 sections: general system description, design specifications, monitoring and oversight, data governance, performance metrics, human oversight measures, cybersecurity, post-market monitoring plan, and the Declaration of Conformity.
The August 2026 enforcement deadline means providers must have a complete technical file ready for inspection by a notified body or market surveillance authority. Most engineering teams don’t know which artefacts they’re missing until an auditor asks.
All 9 Annex IV Sections
- 1General description of the AI system
- 2Detailed description of the elements and development process
- 3Detailed information on monitoring, functioning, and control
- 4Description of the risk management system
- 5Description of changes made throughout the lifecycle
- 6New version listing and update documentation
- 7Technical standards applied and solutions adopted
- 8EU Declaration of Conformity
- 9Post-market monitoring plan
How it works
- 1.Connect your GitHub repo (read-only OAuth) or upload a .zip
- 2.We scan for Annex IV compliance signals across your codebase and documentation
- 3.Get a PDF report with file-level gaps, missing artefacts, and remediation steps per Annex IV section

Why this matters now
- →August 2026: enforcement deadline for high-risk AI providers
- →Most teams discover Annex IV gaps only when an auditor asks
- →A complete technical file takes 2–4 weeks to produce from scratch
- →Early access teams get priority onboarding and direct support during their first scan
Why existing tools aren’t enough
- →Most Annex IV tools today are questionnaire-based — they require 40–80 hours of manual input
- →None scan your actual codebase to identify missing documentation artefacts
- →AI Act Gap is the only tool building automated code-level Annex IV gap detection
