What is Annex IV?

EU AI Act Annex IV (Article 11) defines the mandatory technical documentation that providers of high-risk AI systems must create and maintain. It covers 9 sections: general system description, design specifications, monitoring and oversight, data governance, performance metrics, human oversight measures, cybersecurity, post-market monitoring plan, and the Declaration of Conformity.

The August 2026 enforcement deadline means providers must have a complete technical file ready for inspection by a notified body or market surveillance authority. Most engineering teams don’t know which artefacts they’re missing until an auditor asks.

All 9 Annex IV Sections

  1. 1General description of the AI system
  2. 2Detailed description of the elements and development process
  3. 3Detailed information on monitoring, functioning, and control
  4. 4Description of the risk management system
  5. 5Description of changes made throughout the lifecycle
  6. 6New version listing and update documentation
  7. 7Technical standards applied and solutions adopted
  8. 8EU Declaration of Conformity
  9. 9Post-market monitoring plan

How it works

  1. 1.Connect your GitHub repo (read-only OAuth) or upload a .zip
  2. 2.We scan for Annex IV compliance signals across your codebase and documentation
  3. 3.Get a PDF report with file-level gaps, missing artefacts, and remediation steps per Annex IV section

Your code is never stored. Scanned in memory, deleted immediately. Read-only GitHub access only.

We review applications within 1–2 business days. Teams with an active audit deadline are prioritised.

We review applications within 1–2 business days. Teams with an active audit deadline are prioritised.

Why this matters now

  • August 2026: enforcement deadline for high-risk AI providers
  • Most teams discover Annex IV gaps only when an auditor asks
  • A complete technical file takes 2–4 weeks to produce from scratch
  • Early access teams get priority onboarding and direct support during their first scan

Why existing tools aren’t enough

  • Most Annex IV tools today are questionnaire-based — they require 40–80 hours of manual input
  • None scan your actual codebase to identify missing documentation artefacts
  • AI Act Gap is the only tool building automated code-level Annex IV gap detection
